Job Summary
• The primary purpose of this position is to plan, coordinate, monitor and evaluate health care activities for high risk, high cost, and problem prone patients. Insures and facilitates achievement of quality care, appropriate care costing and clinical outcomes.
Expected Duties and Responsibilities
• Responsible for all aspects of Nursing (i.e., administration, education, research and practice)
• Provides oversight for Government Performance and Results (GPRA) clinical objectives for health care, guidance on Electronic Health Record (EHR) clinical applications, and development of models for overall coordinated case management with integration of nursing and related services.
